POWER Help - New Teacher Request Form

Please complete the following form if you would like to add a teacher in POWER for the purposes of supervision and/or evaluation. If you have any questions regarding the form or this process please contact the POWER Helpdesk at power.help@utoronto.ca.

If the teacher is not a physician or faculty appointed, you will be required to upload an email with your PD's approval to add the teacher. Please note, the form cannot be submitted without this information.
