We encourage you to record what happened to you in an organized way, in case you later change your mind. Recording details as close to the event as possible will improve the quality of what you document. Here are some ways that you can do that:
Send Yourself an Email
One option is to send yourself a time-stamped email with the details of the event for your own records. If you want some guidance on what details to include, feel free to refer to our disclosure form to see what prompts are included. Here are some questions that you can consider including in your record (there is no pressure to answer any or all of them, this is just a guide):

Alternatively, you can download an interactive form (in either PDF or word format) to your desktop and complete it privately on your personal computer. Because it will be completed on your computer, we will not have access to any of the data that you record.
