1. Initial University AFC Application
IMPORTANT NOTE:
The University of Toronto will temporarily pause the acceptance of initial AFC program applications, effective November 16, 2026. 🚨
This pause is necessary to focus institutional resources and administrative efforts on preparing for the 2028 University of Toronto on-site accreditation survey. Applications will be accepted until November 15, 2026 and reopen on November 15, 2028. For additional information, please contact pgme.accreditation@utoronto.ca.
To establish an AFC program at the Temerty Faculty of Medicine, University of Toronto, initial university AFC applications are accepted and reviewed three times each academic year.

Please complete and submit the initial University AFC Program Application Form via email to the PGME AFC Project Coordinator (pgme.afc@utoronto.ca). Incomplete applications will not be reviewed and will be deferred to the next review period until all required documents are received.
Applicants can expect a response approximately 3 to 4 weeks after the period/cohort deadline. Successful applications will be forwarded to the PGME Accreditation team for further review and to initiate the next steps.

PGME holds an Information Session before each period/cohort deadline. These sessions offer a brief overview of the governance structure and key accreditation processes relevant to applicants.
To attend a session, please email the PGME AFC Project Coordinator (pgme.afc@utoronto.ca), and they will send you a registration link.
