Initial University AFC Application

There are two types of initial University AFC Application forms:

Single Discipline Entry Pathway – this form is used when only one department is involved
- University AFC Application Form
- Template for Letter of Department Approval
Multidisciplinary Entry Pathway – this form is used when more than one department could be involved
- University AFC Application Form
- Template for Letter of Departments Approval

NOTE: The University AFC Application Form is provided in a protected Microsoft (MS) Word format to ensure proper formatting and that only fillable sections can be completed. As a result, you may encounter some formatting restrictions.
