The Eval Forms Mapping

This screen is where the evaluation forms and rotation services come together or linked to each other. This enables the system to recognize which forms are used per rotation service.

Once an evaluation form is uploaded to POWER by the PGME Helpdesk, the Program Administrator can map or link the form to its rotation service. The evaluation can be linked to one or more rotation services or all rotation services, depending on the evaluation form.

Evaluation forms can be used as of a certain date. This is setup during the time the evaluation form is mapped.
