Learner Support Request Form
Important Notes before completing this request
- Requests for coaching must be made by a Program Director, Supervisor and/or person in a leadership role within the Temerty Faculty of Medicine. Requests from learners will not be accepted.
- The Learner Education Support Unit will work diligently to respond to your request in a timely manner. Please note during busy periods it may take 5-7 business days for a response to your request.
- Coaching availability is dependent on the capacity and the availability of our PGME Coaches. Should the request meet specified criteria below, it is possible that when demand is high a delay may occur with assigning a learner to a coach.
What TYPE of learner support is being requested:
CanMEDS Roles Coaching Support (limited to 2-3 sessions)
Examination Preparation Support (limited to 1-2 sessions)
Important Notes:
To be eligible for exam preparation support, the learner must meet the following criteria:
- The learner has had significant issues with previous examinations, either program-based or national/USA in-training exams or licensure examinations.
- The learner has a formally diagnosed learning disability or other wellness issue that impacts their exam performance
* General interest in improving exam performance or general nervousness about examinations does not fit the criteria for exam support through PGME at this time.
